HOW THE PROGRAM WORKS

  • Take a few minutes to review the supplier list for selection and find those categories in which you purchase incentives.
  • Print out the ballot.
  • On the ballot, circle the corresponding number from the supplier list next to those companies you find superior (up to two per category)
  • Submit your ballot via fax to: 646-654-7365

  • Limit one ballot per person. Additional entries will not be included in tabulation.
  • If you do not see a supplier listed that you would like to vote for, you will have the opportunity to write-in their name on the ballot
  • All ballots will be verified and tabulated independently
